29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard.    Employees were exposed to fall hazards. The platform located above Foam Injection Press #4 was missing an area of guardrail resulting in an open side. When employees are working above dangerous equipment all sides of the platform must be guarded in order to prevent employees from falling into or onto the dangerous equipment.
