On or about October 10, 2017, during the installation of roofing material at the job site located at 610 Lynwood Ave SW, Canton, Ohio 44710; the employer did not ensure employees working on the roof of a commercial location were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.
