29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.     On or about October 10, 2017, during the installation of roofing material at the job site located at 610 Lynwood Ave SW, Canton, Ohio 44710; the employer did not ensure employees working on the roof of a commercial location were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:     On or about October 10, 2017, during the installation of roofing material at a job site located at 610 Lynwood Ave SW, Canton, Ohio 44710; the employer did not ensure employees working on a roof of a commercial location were protected from a fall of approximately 18.8 feet by utilizing a fall protection system.
