29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:     The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This most recently occurred on October 12, 2017 on the jobsite located at 2509 Pinehurst Cir, Papillion, NE 68133.  Four employees were observed on the roof of a single-story house with a 6 in 12 steep sloped pitch at heights greater than 6 feet above the ground and not utilizing any form of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     The employer is failing to ensure that each employee exposed to fall hazards has been effectively trained on fall hazard recognition and on how to use fall protection.  This most recently occurred on October 12, 2017 on the jobsite located at 2509 Pinehurst Cir, Papillion, NE 68133.  The employer has 4 employees who have not received fall protection training and are exposed to fall hazards while re-roofing an existing single-story house with a 6 in 12 steep sloped pitch and a ground to eave height of about 9.5 feet.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder was not extended at least 3 feet (0.9 m) above the upper landing surface being accessed and a grasping device was not provided to assist employees in mounting and dismounting the ladder:       The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof.  This most recently occurred on October 12, 2017 on the jobsite located at 2509 Pinehurst Cir, Papillion, NE 68133.  Employees are using an aluminum extension ladder to enter and exit the roof.  The ladder was secured at the top; however, it was not extended to at least 3 feet above the landing surface and no grabrail was provided to assist in mounting and dismounting.
