29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about October 11, 2017, at the sheeting machine, the belt on the conveyor going to the oven was not guarded. The conveyor belt is loose and presents an ingoing nip point. The employees will work near and inspect the food product on the line. The employees are exposed to the hazard of being caught-in the rotating conveyor belt. b) On or about October 11, 2017, at the sheeting machine, the dough reclaim conveyor belt was not guarded on the one side. The employees work near and inspect product on the line near the rotating reclaim dough conveyor. The employees are exposed to the hazard of being caught-in the rotating conveyor belt.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: a) On or about October 11, 2017, at the sheeting machine, the rotating shaft near the motor to the flywheel for the cross roller was not guarded. There was an unprotected rotating keyway shaft that has the key in it, that was not guarded. Employees work in the area when inspecting the machine and the status of the product. Employees exposed to the hazard of being caught-in rotating shaft. b) On or about October 11, 2017, at the sheeting machine, the rotating shaft at the dough reclaim conveyor was not guarded. Employees work and inspect in this area. Employees exposed to the hazard of being caught-in rotating shaft.

The shafts projected out more than one half the diameter of the shaft. The employees work and inspect products on the sheeting machine. Employees exposed to the hazard of being caught-in the rotating shaft.
