SGL Carbm LLC - St. Mary's - On or about 10/4/17, and at times prior to, employees were not utilizing lockout/tagout during the changing of chucks on a CNC lathe serial #182262.  An accident occurred on 10/4/17, when an employee accidentally stepped on the foot pedal causing the machine to cycle unexpectedly.  The employee sustained fingertip amputations on his left hand.

Items stored in front of the panel box included, but was not limited to, a 55 gallon trash can.
