29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A.) SGL Carbm LLC - St. Mary's - On or about 10/4/17, and at times prior to, employees were not utilizing lockout/tagout during the changing of chucks on a CNC lathe serial #182262. An accident occurred on 10/4/17, when an employee accidentally stepped on the foot pedal causing the machine to cycle unexpectedly. The employee sustained fingertip amputations on his left hand.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage: A.) SGL Carbon LLC - St. Mary's; fuel cell area; #7 - On or about 10/12/17, and at times prior there to, items were stored in front of a 240 volt electrical panel box. Items stored in front of the panel box included, but was not limited to, a tote of scrap parts. B.) SGL Carbon LLC - St. Mary's; Lathe/Machining Dept. - On or about 10/12/17, and at times prior there to, items were stored in front of a 240 volt electrical panel box. Items stored in front of the panel box included, but was not limited to, a 55 gallon trash can.
