29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a. On or about October 12, 2017, on a 9 foot tall work platform at station #7, employees applying sheathing were exposed to a 9 foot fall hazard because the platform had a 1.5 foot gap from the leading edge of work surface. b. On or about October 12, 2017, on the leading edges of the mezzanine at station #7, there were no guardrails on the platform, exposing employees to a 9 feet fall hazard. c. On or about October 12, 2017, on the leading edges of the mezzanine catwalks at station #8, there were no guardrails on the platform, exposing employees to a 9 feet fall hazard.

29 CFR 1926.102(a)(1): The employer does not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a. On or about October 12, 2017 one employee performing framing work was not wearing appropriate eye protection while using pneumatic nail guns, exposing themselves to eye injuries from flying debris or nails.
