29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed: (a) A.G. Roofing Inc. Located at 107 Denison Placer Rd. Breckenridge, CO 80424: On and preceding October 12 2017, The employer did not ensure that ladders used to access an upper landing surface had side rails extended at least three feet above the upper landing surface. Employees were using an extension ladder to access the roof with approximately 1' extending above the landing surface, and without securing the ladder. Another employee was working off of the ladder to hand panels up to the worker installing the roof. This condition exposed employees to fall hazards from height.
