29 CFR 1926.102(a)(1):  The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a)  M & GR Construction LLC located at 107 Denison Placer Rd. Breckenridge, CO 80424:  On and preceding October 12th 2017, the employer did not ensure that each affected employee uses appropriate eye or face protection when operating pneumatic nail guns at approximately 120 psi.  An employee was installing plywood sheathing on a two story, steep pitched roof using a pneumatic nail gun without eye protection.  This exposed employees to a projectile hazard to the eyes.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     (a)  M & GR Construction LLC located at 107 Denison Placer Rd. Breckenridge, CO 80424:  On and preceding October 12th 2017, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net system, or personal fall arrest systems, nor were employees provided with an alternative fall protection measures.  Employees were sheathing a two story, steep pitched roof without the use of fall protection.  This condition exposed employees to falls from height of approximately 20 feet.
