29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were de-energized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): On or about October 13, 2017, located in warehouse 2 near product aisle 2F15 where employees were exposed to electrical hazards when working near a de-energized electrical cabinet and switch that was damaged without locking or tagging out the disconnect.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial forklift: On or about October 13, 2017, at the facility where employees are exposed to struck-by and crushed-by hazards when operating Nissan forklifts for moving commodities without ensuring employees completed forklift training.
