29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: (a) On or about 10/12/2017, an employee was exposed to cave-in hazards while connecting a water lateral in a trench without adequate protection. Badger Underground Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(c)(2)(I), which was contained in OSHA inspection number 1174810, citation number 1, item number 1 and was affirmed as a final order on 8/17/17, with respect to a workplace located at 4715 Elkhart Ave, Whitefish Bay WI 53217. Badger Underground Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(c)(2)(I), which was contained in OSHA inspection number 973972, citation number 1, item number 2 and was affirmed as a final order on 6/27/14, with respect to a workplace located at 6221 Lake Drive, Whitefish Bay WI 53217.
