29 CFR 1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement:   a) Exterior front, and side of Building: The post on the pump jack scaffold was not plumb and securely braced, thus exposing employees to scaffold collapse. The scaffold was set up on a uneven side walk without being plumb or braced, on or about 10/13/2017

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a) Exterior Front of Building: Employees engaged in siding work were exposed to a fall hazard, at an elevated height of 14 feet above the ground surface. Employees were working without appropriate fall protection  from a pump jack scaffold without side rails installed, on or about 10/13/2017

29 CFR 1926.452(j)(2): Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled.  a) Exterior Building Front: Employees performing work on a pump jack were exposed to a fall hazard  of approximately 14'. The scaffold's poles were not secured at the bottom, on or about 10/13/2017
