29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a) On or about October 3, 2017 and at times prior thereto, employees were working to re-roof a frozen and slippery roof with a ground to eave height of approximately 16.5 feet and slopes of 5 in 12 to 8 in 12. A personal fall arrest systems was not used and those provided were not sufficient to protect all workers from falls. Note: Abatement certification AND supporting documentation are required for this item.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards. (a) On or about October 3, 2017, and at times prior thereto, Spanish speaking workers engaged in re-roofing a 16 to 18 feet high roof had not received training sufficient so they could understand how to properly and safely use the personal fall arrest systems. Note: Abatement certification AND supporting documentation are required for this item.
