29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about October 13, 2017, during the installation of water and sewer lines across from 4340 W. Streetsboro Road, Richfield, Ohio, the employer did not ensure an employees head was protected by a protective helmet (hard hat) were there was a danger of head impact from tools and/or clumps of dirt placed and/or positioned less than one (1) foot above the edge of the trench.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about October 13, 2017, during the installation of water and sewer lines across from 4340 W. Streetsboro Road, Richfield, Ohio, the employer did not ensure an employee working inside a trench, at a depth of approximately 6 feet and with near 90 degree vertical walls, was protected from cave-ins by an adequately designed protection system. The soil was classified as; Cohesive Soil, type B.
