29 CFR 1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of the chop saw: (a) Facility, 135 Roaring Run Road, Champion, PA: On or about September 29, 2017 the point of operation on the Precision Products chop saw, Model 18MLH, Serial #696 was not adequately guarded to prevent contact with the rotating blade. An operator sustained an amputation to their right middle finger to the first knuckle when cutting dowels. The guard consisted of a fixed "u" shaped bar that was positioned approximately 6 1/2" above the table and did not prevent an operator from being able to place their hands in the danger zone.
