29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest: a. On or about October 11, 2017, two employees working on the roof were exposed to a twenty feet fall hazard because there was no guardrail system, safety net system, personal fall arrest system or safety monitor system being used.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about October 11, 2017, stairway 2nd floor, back of building, employees were using a 20 feet extension ladder to gain access and exit from the roof. The ladder was not extended 3 feet and secured to prevent accidental displacement and employees were exposed to a 10 feet fall hazard.
