On or about October 11, 2017, roof area, two employees working on the roof were not protected from falling 20 feet to the ground by the use of a guardrail system, safety net system, or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about October 11, 2017, stairway 2nd floor, back of building, employees used a 20 feet extension ladder that was not extended at least 3 feet above the upper landing surface to gain access and exit the roof, being exposed to a 10 feet fall hazard.
