29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:    (a)   PRODUCTION AREA, BERTOCCHI MACHINE, PUMP - Sanitation employee(s) disassembled the drain pipe for the holding tank at the bottom of the Bertocchi extraction machine in order to get the tank to drain quicker.  Disassembly of this piping was not part of the established procedures to turn off and de-energize the machine.  This exposed an employee to amputation injuries when the employee attempted to reach into the opening of the still operational Ampco model ZP1-130-50 Type Waukesha model 130 pump connected and adjacent to the drain pipe in order to retrieve a gasket/seal that had fallen into the pump after he had disassembled the pipe that was feeding into the pump, on or about October 6, 2017.  No abatement certification or documentation required.
