29 CFR 1910.134(e)(1):  The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:                           a) On or about October 4, 2017, at the above located workplace, an employee working with various chemicals was required to wear a tight fitting respirator. The employer did not provide medical evaluations to determine the employees' ability to use a respirator.

29 CFR 1910.1200(e)(1): Employers did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met:               a) On or about October 4, 2017, at the above located workplace, employees worked with Easter Tech, LLC., FBX-100, Pretreatment Solution; W.M. Barr, Goof Off, Pro Strength Remover (product Number FG653) and other chemicals. The employer did not develop and implement a written hazard communication program. Employees were thereby exposed to the hazards associated with chemical exposure.

29 CFR 1910.1200(h):  Employees were not provided information and training as specified in 29 CFR 1910.1200(h)(1) and (2) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area:                 a) On or about October 4, 2017, at the above located workplace, employees worked with Easter Tech, LLC., FBX-100, Pretreatment Solution; W.M. Barr, Goof Off, Pro Strength Remover (product Number FG653) and other chemicals. The employer did not provide employees information and training as specified in 29 CFR 1910.1200(h)(1) and (2) on hazardous chemicals in their work area. Employees were thereby exposed to the hazards associated with chemical exposure.

Barr, Goof Off Pro Strength Remover (product Number FG653) and other chemicals. The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done. Employees were thereby exposed to the hazards associated with industrial strength chemicals.
