29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Packing Department: The heat sealer (pressure bar) and the sealant element on the T-1000 Advanced Poly-Bagger (S/N: 208040850) was equipped with an inadequate guard that had an opening of approx. 5 inches long by 8.5 inches wide which allowed employees to place their hands into the point of operation, exposing employees to an amputation injury, on or about October 5, 2017.
