29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees.  On or about October 12, 2017, the employer exposed employees to an electrical  hazard. The employer allowed employee to use a wet vacuum to vacuum up water from the concrete foundation. The wet vacuum was connected  to a yellow and black electrical extension cord that had yellow electrical tape wrapped around a section of the electrical cord.
