29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  a) Jobsite: Bellwood Manor Apartments & Townhomes; Building#200; Bellwood Drive, North Hills Ross Twp, Pittsburgh PA- On or about October 2, 2017, employees were working unprotected and exposed to fall hazards of between approximately 20 feet to 22 feet while removing shingles on a 4/12 sloped roof of greater than 50ft width. Employees were not protected by guardrail systems, safety net system, or personal fall arrest system.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:   a) Jobsite: Bellwood Manor Apartments & Townhomes; Building#200; Bellwood Drive, North Hills Ross Twp, Pittsburgh PA-On or about October 2, 2017, employees were working unprotected and exposed to fall hazards while removing shingles.  The employer did not maintain a written certification record of fall protection training for each employee working on the 4/12 pitch roof with approximately 22 feet fall hazards from roof to the ground.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:    a) Jobsite: Bellwood Manor Apartments & Townhomes; Building#200; Bellwood Drive, North Hills Ross Twp, Pittsburgh PA- On or about October 2, 2017, employees accessed the 22-feet height roof by use of a portable ladder that was not extended at least 3 feet above the upper landing surface of the roof.
