On or about October 09, 2017 the employer reported an amputation incident that initially occurred on July 14, 2017. One (1) employee was involved in a partial amputation of left hand.

29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut. On or about July 14, 2017, at the center of the warehouse, the employer failed to have a protective guard on a Jet 10" Tilting Saw (Serial #12081234). One (1) employee had left hand amputated and the employer did not report amputation until October 09, 2017.
