29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    Employees are exposed to occupational noise above the action level. The employer is failing to protect employees from occupational noise exposure by effective administration of a hearing conservation program. 8-hour time-weighted average sampling during the sampling period on December 20, 2017 showed decibel levels at or above a dose of 50 percent on the A scale or an equivalent sound level of 85 decibels.    a) An employee working at or near the edger saw was exposed to continuous noise levels at 260.1% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 96.8 dBA. The sampling was performed for 460 minutes during one shift, exposure calculations included a zero time for the 20 minutes not sampled.    b) An employee working at the tie grader was exposed to continuous noise levels at 273.3% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 96.5 dBA. The sampling was performed for 464 minutes during one shift, exposure calculations included a zero time for the 16 minutes not sampled.    c) An employee working at or near the stacking line was exposed to continuous noise levels at 151.1% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 92.9 dBA. The sampling was performed for 461 minutes during one shift, exposure calculations included a zero time for the 19 minutes not sampled.

29 CFR 1910.265(c)(24)(viii)(c): Wire rope clips attached with U-bolts did not have the bolts on the dead or short end of the rope:    Employees are exposed to struck-by and crushing hazards. One of the two wire rope clips attached with the U-bolts was installed backwards compromising the integrity of lifting equipment. This was discovered in the mill on the running line on the overhead gantry crane. The gantry crane is used to lift and transfer large logs to the mill to be cut.

29 CFR 1910.265(c)(24)(viii)(e): Fittings. Hooks, shackles, rings, pad eyes, and other fittings that show excessive wear or that have been bent, twisted, or otherwise damaged shall be removed from service.  Employees were exposed to struck-by or crushing hazards. In the mill an overhead gantry crane is being used to transfer large tree stumps to be cut. Hooks on the hoist were damaged and missing their safety clasps.  a) The hook located at the end of the hoist rope was missing a safety clasp and could become dislodged from the hoist mount while carrying material.   b) The hook attached to the pulley wheel  for the log tongs was missing a safety clasp and could become dislodged from the hoist while carrying material.

Listed or labeled relocatable power taps were not used or installed in accordance with instructions included in the listing or labeling which were in operation at 120 V. In the utilities area a relocatable power tap was connected in series (daisy chained) from a receptacle to power a portable lamp and lasers for the edger machine.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    Employees were exposed to electrical shock hazards. In the facility flexible cord and cables were used as a substitute for permanent wiring. This was demonstrated in the mill  between the mechanical room and the chipper room. An extension cord was run from an outlet receptacle through the ceiling rafters to power machinery in the chipper room.

In the mill a gantry crane is being used to lift timber logs with a hoist and log tongs. The beam that the hoist is attached to does not contain the rated capacity for the maximum load it can safely handle.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical    Employees were exposed to a chemical  known as "used oil" in the workplace. Secondary containers were being used to dispense the hydraulic oil as a lubricant. The containers either did not contain a label at to their contents or they contained a label that did not match their contents.
