29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    a) King George Auto Sales Inc. dba All American Auto Salvage/192 Leesville Ave/Rahway, NJ - Yard: Forklift operators were not trained and evaluated to safely operate powered industrial trucks at the worksite.  To include, but not limited to:  -Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.      Condition noted on or about 10/17/2017.

29 CFR 1910.178(l)(2)(ii): Training did not include an evaluation of the operator's performance in the workplace:  a- King George Auto Sales Inc. dba All American Auto Salvage/192 Leesville Ave/Rahway, NJ - An employee, who was trained by a previous employer in forklift operations, was not evaluated by the current employer, to ensure the effectiveness of the training received.  The employer must certify that each operator was trained and evaluated.  The certification is to include, but not limited to, the name of the operator, the date of the training or evaluation and the name of the person providing the training or evaluation.  Condition noted on or about 10/17/17.
