29 CFR 1910.178(p)(1): Powered industrial truck(s) with defect(s) or in any way unsafe were not withdrawn from service until restored to safe operating condition(s):    2450 Adie Road, Suite 100, Maryland Heights: Employer is exposing employees to hazardous conditions.  The employees are permitted to operate forklifts with defects.     On and about 10/17/2017 employees operated forklifts which were not removed from service until defects such as, but not limited to, broken brake and signal lights, non-legible identification plates, and oil leaks were corrected.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks:   2450 Adie Road, Suite 100, Maryland Heights: Employer is exposing employees to hazardous conditions.  The employees are permitted to operate a drill press without a guard to prevent contact with the rotating parts.   On and about 10/17/2017 employees operated a Clausing drill press which was not guarded to prevent employee contact with rotating chuck and drill bit.

29 CFR 1910.1020(g)(1)(i): The employer did not inform current employees upon their first entering into employment and at least annually thereafter, of the existence, location, and availability of any records covered by 29 CFR 1910.1020:   2450 Adie Road, Suite 100, Maryland Heights:  Employer is exposing employees to potential hazardous conditions.  Employees work where they are not provided information about their rights of access to medical and exposure records.   On and about 10/17/2017 employees were not informed of the existence of medical and exposure records, the location of the records, and the availability of the records.
