29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a) At 310 Samford Village Court, Auburn, Alabama: On or about October 17, 2017 and at times prior; the employer allowed his employees to work from a platform being lifted by a powered industrial truck and the platform was not secured to the powered industrial truck exposing his employees to a fall hazard of 22 feet to the ground below.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) At 310 Samford Village Court, Auburn, Alabama: On or about October 17, 2017 and at times prior; the employer allowed his employees to work from a platform being lifted by a powered industrial truck without fall protection exposing his employees to a fall hazard of 22 feet to the ground below. b) At 310 Samford Village Court, Auburn, Alabama: On or about October 25, 2017 and at times prior; the employer allowed his employees to work from a platform being lifted by a powered industrial truck without fall protection exposing his employees to a fall hazard of 14 feet 5 inches to the ground below.
