29 CFR 1910.107(b)(10): When spraying areas are illuminated through glass panels or other transparent materials, only fixed lighting units shall be used as a source of illumination. Panels were not effectively isolating the spraying area from the area in which the lighting unit is located, and were not of a noncombustible material of such a nature or so protected that breakage would be unlikely. Panels shall be so arranged that normal accumulations of residue on the exposed surface of the panel will not be raised to a dangerous temperature by radiation or conduction from the source of illumination.    On October 17, 2017, and times prior thereto, in the spray paint booth, broken, damaged glass panels were used ineffectively isolating the spraying area from which the lighting unit was located.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:      On October 17, 2017, and times prior thereto, at this facility, the employer had not established or implemented a respiratory protection program when employees were required to wear tight-fitting respirators when using the hazardous chemicals and materials for mixing, spraying, painting, sanding and grinding the automotive parts.    An effective respiratory protection program would include, but not limited to:    a. Procedures for selecting respirators for use in the work place;  b. Medical evaluation of employees required to wear respirators;  c. Valid fit testing procedures for each type of tight-fitting respirators;  d. Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding and maintaining respirators; and,  e. Training of employees in the proper use of respirators.
