This condition resulted in employees being exposed to a caught-by hazard.    b) On or about 9/11/2017, the employer did not ensure ensure control of the hazardous electrical energy prior to servicing or maintaining machinery. Employees performed maintenance on the Doboy Model Scotty II Horizontal Flow Wrapper SN 96-17699. This condition resulted in employees being exposed to an amputation hazard.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
