29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: a) On or about September 16, 2017, a Machine Operator Trainee received a hand injury while removing material from the "Goober Pad" under the Die Lip on Line #26. The Machine Operator Trainee was injured when 3 fingers of his left hand were caught in the Die Roll Stand ingoing nip point. The employer had not developed and implemented energy control procedures or provided alternative measures which provide effective protection for employees removing material from the "Goober Pad" or replacing the "Goober Pad" during production runs. b) On or about October 18, 2017, the employer had not developed and implemented energy control procedures or provided alternative measures which provide effective protection for Line 21, 22, 23, 24, 25, 26, 27, 28 and 29 Machine Operators adjusting the Die Width Bolts and cleaning excess material from die lips from the Die Roll Stand ingoing nip point. c) On or about November 30, 2017, the employer had not developed and implemented energy control procedures or provided alternative measures which provide effective protection for Machine Operators adjusting circular blades, adjusting the collars on the idler roller and changing the trim material width near the Line 21, 22, 24, 25, 27, 28 and 29 Maxon ingoing nip points when making production changes.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: a) On or about October 18, 2017, the Line #26 Die Roll Stand framing and the Die did not prevent Line Operators walking by from accessing the Die Roll Stand ingoing nip point.
