Section 5(a)(1) of the Occupational Safety and Health Act of 1970:    The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing, caught between and struck-by hazards.  The most recent example of this was found on October 18, 2017 at the facility located at 4870 S. 137th Street. Omaha, NE; where the employer is using unsafe work practices associated performing repairs to vehicles elevated by automotive lifts.  Instances of unsafe work practices include but are not limited to:   1.     Allowing employees to perform maintenance on vehicles elevated by a four-post drive where the safety latches preventing catastrophic failure have been modified or removed.    2.     The employer is failing to provide and document operator training on the safe use and operation of automotive lifts using manufactures  operators manuals and ANSI/LI ALOIM: 2008 standards.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: The employer is failing to protect employees from struck by hazards associated with operating a bench grinder without a tool rest.  The most recent occurrence of this was evident during the inspection of the facility located at 4870 S. 137th ST. Omaha, NE; where the employer has employees using the K-T Industrial bench grinder without a tool rest to sharpen tools and remove rough edges while maintaining customers vehicles.

29 CFR 1910.305(b)(2): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  The employer is failing to protect employees from electrical hazards associated with missing covers to outlet boxes.  The most recent occurrence of this was evident during the inspection of the facility located at 4870 S. 137th ST. Omaha, NE; where the employer has employees maintaining equipment in close proximity to the outlet box with a missing cover located on the wall adjacent to the air compressor.

Omaha, NE; where the employer has employees exposed to the damaged plug for the tubing bender machine.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):                                                         The employer is failing to protect employees from caught between and struck by hazards associated with maintain equipment that requires the application of a lock or tag to isolate the hazardous stored energy.  The most recent occurrence of this was evident during the inspection of the facility located at 4870 S. 137th ST. Omaha, NE; where the employer has employees maintaining customers vehicles without having developed a written lockout and/or tag out program as means of identifying hazards and mitigating risks.  Instances of hazards and exposures include but are not limited to:  i.      Employees exposed to caught between hazards while conducting vehicle engine repairs.  ii.     Employees exposed to crushing hazards associated to conducting automotive repairs while working under the cars.  iii.    Employees exposed to shock hazards associated to conducting automotive electrical repairs.
