29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A. On or about February 26, 2018, near Coolant Storage, Lower Level: On the Daito GA320 horizontal metal-cutting band saw, a 4 1/2" unused portion of the blade to the left of the point of operation was unguarded during operation, exposing operators to amputation hazards. A yellow adjustable blade guard was present on the machine, but had not been moved into place to cover the moving blade.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A. On or about February 26, 2018, Lathe & Mill Area: In the following instances, CNC equipment was operated without interlocks engaged, allowing for entry into a hazard area created by rotating parts: 1. Hurco VM2 CNC Vertical Machining Center: Enclosure doors were able to be opened during spindle operation. 2. Hitachi Seiki Hitec Turn 25, Type HT-25, S/N 25180-SC: Enclosure doors were able to be opened during spindle operation. If the doors were already open, the equipment would not start. The H & B Tool & Engineering Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1016943, citation number 1, item number 1 and was affirmed as a final order on March 18, 2015, with respect to a workplace located at 481 Sullivan Avenue, South Windsor, Connecticut.

On or about February 26, 2018, Lathe and Mill Department, near the Kia Center H40 CNC machining center: A 2-receptacle wall-mount listed electrical outlet box with perforated knockouts was provided to provide pendant power to equipment in the area, including a 31" King of Fans metal-encased pedestal fan.
