29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (1) Preparation for shutdown, (2) Machine or equipment shutdown, (3) Machine or equipment isolation, (4) Lockout or tagout device application, (5) Stored energy (6) Verification of isolation. a. On or about October 18, 2017, located on Line 1, employees were exposed to machine hazards associated with moving parts and ingoing nip points when they engaged in in removing stuck/jammed pieces of wood in a Lewco live roller conveyor where an associated zone of danger existed. The employer did not ensure that employees implemented energy control application steps prior to allowing them to perform lockout activity.

29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device. a. On or about October 18, 2017, located on Line 1, 6-10 employees would perform cleaning in and around a Lewco live roller conveyor without a group lockout being performed so that all authorized employees had control over their own safety, thereby exposing the employees to the unexpected startup or release of stored energy of the conveyor.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. a. On or about October 18, 2017, located on Line 1, the employer did not ensure that the conductors entering a terminal box on a Leeson electrical motor were protected from damage. The conduit surrounding the conductors was damaged which revealed the insulation to the conductors.
