Kay Metals: On or about October 18, 2017 and at times prior, employees were hoisting 78 inch outside diameter kettles with a SkyJack SJ643TH forklift.  The SkyJack SJ643TH forklift has a capacity of 6000 lbs. and the 78 in. outside diameter kettles weigh 9300 lbs.    b) P. Kay Metals: On or about October 18, 2017 and at times prior, employees were hoisting 108 inch outside diameter kettles with a Komatsu 40 forklift.  The Komatsu 40 forklift has a capacity of 9000 lbs. and the 108 inch outside diameter kettles weigh 14800 lbs.       Note: Abatement certification and documentation is required for this item.

Kay Metals: On or about October 18, 2017 and at times prior, employees were entering kettle setting shells to install insulation.  Employees are entering the space via a removable ladder.  The space is a confined space since it is large enough to bodily enter, not intended for human occupancy, and has limited means of egress.  The space is not a permit required confined space.
