29 CFR 1910.132(d)(1): Hazard assessment and equipment selection. The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). On or about October 19, 2017, the employer did not adequately conduct a hazard assessment to determine what personal protective equipment to be utilized by employees during the automated parcel bundle sorter (APBS) number 2 blue dumper operation. Employees were exposed to struck by hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about October 13, 2017, in the MPA number 2, the employer did not ensure employees were protected from being struck by mail being dumped by the APBS number 2 machine. Employees were exposed to struck by hazards.
