29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized, other safety related work practices were not used to protect employees who could be exposed to the electrical hazards involved: a) Transformer T-97; On or about September 22, 2017 and at time prior thereto, the employer exposed employees to electrical hazards in that the qualified employee in charge did not ensure that employees used the required electrical protective equipment when checking the phase rotation on a live 2,400v transformer. b) Transformer T-12; On or about September 22, 2017 and at time prior thereto, the employer exposed employees to electrical hazards in that the qualified employee in charge did not ensure that employees used the required electrical protective equipment when checking the phase rotation on a live 480v transformer.
