29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards when conducting roofing operations on residential structures.  This was most recently documented at 1525 Applewood Dr., Papillion, NE on or about October 18, 2017 when employees were observed installing shingles to a residential structure while working greater than 6 feet above the ground without fall protection in use.

This was most recently documented at 1525 Applewood Drive, Papillion Nebraska on or about October 18, 2017 when 3 employees were observed using a ladder with side rails that were not supported equally.
