29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling: The employer is failing to protect employees from a fall hazard when conducting roofing work. This was most recently documented at 650 N 109th St. Omaha, Nebraska on or about October 19, 2017. Employees were observed removing shingles while working on the sloped roof at a height greater than 25 feet from the ground below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. The employer is failing to protect employees from a fall hazard when using an extension ladder to access a roof that was not extended 3 feet above the landing surface. This was most recently documented at 650 N 109th St. Omaha, Nebraska on or about October 19, 2017. Employees were observed accessing the roof by the extension ladder. This ladder was not extended 3 feet above the eve as is required by this subpart.
