29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a)  5555 Stapleton Dr. (Montgomery, AL);  On or about October 4, 2017 and at times prior thereto, employees using pneumatic nail guns and circular saws were exposed to eye injuries from debris and nails in that the employer did not ensure eye protection was being used.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a)  5555 Stapleton Dr. (Montgomery, AL);  On or about October 4, 2017 and at times prior thereto, employees working from a scaffold engaged in cornice installation were exposed to fall hazards of 10 feet 6 inches to the ground below in that the employer did not ensure fall protection was being used.
