(a) 107 RHL Boulevard: On or about October 14, 2017, and times prior, the General Contractor employer did not ensure the use of protective foot wear by subcontractor employees required to perform work such as wood framing and demolition activities.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters (GFCI) for personal protection. (a) 107 RHL Boulevard: On or about October 14, 2017, and times prior, the General Contractor authorized subcontracted employees to use hand tools powered by a 120 volt receptacle that was not protected by a ground fault circuit interrupter.

(a) 107 RHL Boulevard: On or about October 14, 2017, and times prior, the general contractor did not ensure the electrical service to the building was shut off, capped or controlled prior to allowing subcontracted employees to remove internal walls, drop ceilings, drywall, and the external roofing structures.
