On or about 10/17/2018 at the 1445 New Britain Avenue Construction Project, employees were using a warning line system as fall protection and the warning line sagged below the minimum 34 inches.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: On or about 10/17/2017 and 10/18/2017 at the 1445 New Britain Avenue Construction Project, employees were stepping up to and down from the top of the parapet wall to access the ladder and the height was greater than 19 inches.
