29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  (a)  Jobsite:  On or about October 12, 2017, an employee was working on a residential roof and he was not protected from falling to the ground.  The fall hazard was approximately 8 1/2 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) Jobsite:  On or about October 12, 2017, the employer did not train an employee in potential fall hazards and how to minimize them when working from a roof.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA:   (a) Jobsite:  On or about October 12, 2017, an employee was admitted to a hospital as a result of work related injuries and the employer did not notify OSHA of the hospitalization.
