29 CFR 1910.305(a)(2)(i): Temporary electrical power and lighting installations rated 600 volts, nominal, or less were used for purposes other than those permitted in subparagraphs (A), (B) and (C) of this paragraph:   On or about October 18, 2017 and times prior thereto, in the Motor Control Center (MCC) Room, where employees were exposed to a 480 VAC electrical shock hazard when operating circuit breaker handles with temporary wiring used in a prohibited manner.

29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed:  On or about October 18, 2017, and times prior thereto:  a. In the Motor Control Center (MCC) Room at panel #101 where employees were exposed to an electrical shock when operating circuit breaker handles without the panel being effectively closed.    b. In the MCC Room at panel #102 where employees were exposed to an electrical shock when operating circuit breaker handles without the panel being effectively closed.    c. In the MCC Room at panel #104 where employees were exposed to an electrical shock when operating circuit breaker handles without the panel being effectively closed.    d. In the MCC Room at panel #105 where employees were exposed to an electrical shock when operating circuit breaker handles without the panel being effectively closed.    e. In the MCC Room at panel #106 where employees were exposed to an electrical shock when operating circuit breaker handles without the panel being effectively closed.    f. In the MCC Room at panel #107 where employees were exposed to an electrical shock when operating circuit breaker handles without the panel being effectively closed.    g.
