OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards while using a damaged pendant controller for an overhead under hung hoist: (a) Crane 6, serial number EC3G0407XV; On or about October 19, 2017, the function labels on the pendant control station were not legible. (b) Crane 6, serial number EC3G0407XV; On or about October 19, 2017, the employer did not replace the damaged pendant on the overhead hoist after it was determined to be a hazard by a qualified person.
