29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems. On or about October 19, 2017, three employees were installing scaffolding on the rooftop of the commercial property under construction up to approximately 11 feet high without fall protection, exposing the employees to a fall hazard.

29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards: On or about October 19, 2017, and at times prior thereto, employees working on the commercial property under construction had not received training on the use of fall protection prior to being allowed to work in elevated places, exposing the employees to fall hazards up to approximately 11 feet.
