29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 09/27/2017 at the job site at 3200 Town and Country, in St. Charles, MO, employees working on roof engaged in removing/replacing  shingles,  were not provided conventional fall protection. The ground to eave height was approximately 9' 8".

29 CFR 1926.503(a)(1): The employer does not ensure that a training program for each employee potentially  exposed to fall hazards to enable each employee to recognize the hazards of falling and  the procedures to be followed in order to minimize these hazards.  On or about 10/03/2017, at the jobsite at 3200 , St Charles, MO, an employee engaged in shingle removal, fell approximately 9' 8" off of a roof.  The employer failed to provide the employee with training on requirement to have fall protection when exposed to fall hazards of 6' or more.
