29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:    a)  Andersen's Sales & Salvage, Inc. 1490 E. 8th Street, Greeley, CO  80631:  On and before October 12, 2017, although the employer had implemented some procedures to control hazardous energy, the procedures did not fully protect employees from hazardous energy when employees were replacing the rotor, hammers, and pin protectors on the MegaShredder model 88x112 M-Series hammer mill.  The locking pin that held the rotor in position was removed several times during this process to allow the rotor to rotate to a balanced position.  When the locking pin was removed, and the rotor was rotating, the employer did not develop, document, and utilize adequate energy control procedures for the protection of the employees performing this task.  An employee was injured when he was caught in-between the rotating rotor and a working platform.  This condition resulted in exposure to rotating parts, ingoing nip points, and amputation hazards.

29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components:   a)  Andersen's Sales & Salvage, Inc. 1490 E. 8th Street, Greeley, CO  80631:  On and before October 12, 2017 the employer did not remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components.  The employer was replacing the rotor, hammers, and pin protectors on the MegaShredder model 88x112 M-Series hammer mill.  The locking pin that held the rotor in position was removed several times during this process to allow the rotor to rotate to a balanced position.  When the locking pin was removed, and the rotor was rotating, the employer did not ensure that employees were removed from the working platforms located adjacent to the rotor.  An employee was injured when he was caught in-between the rotating rotor and a working platform.  This condition resulted in exposure to rotating parts, ingoing nip points, and amputation hazards.

29 CFR 1910.179(l)(3)(i): Unsafe conditions disclosed during the frequent or periodic inspections required by 29 CFR 1910.179(j) were not corrected before operation of the cranes was resumed:  a)  Andersen's Sales & Salvage, Inc. 1490 E. 8th Street, Greeley, CO  80631:  On and before October 12, 2017 the employer did not ensure that unsafe conditions disclosed during the periodic inspections required by 29 CFR 1910.179(j) were corrected before operation of the cranes was resumed.  The periodic inspections for cranes with serial numbers TC069790A and TC081874A included deficiencies such as, but not limited to, damaged sheaves, broken wire rope, missing bumpers, worn brakes, and a missing warning device.  These unsafe conditions were noted on the crane inspections occurring 8/3/2017, and were not yet repaired by March 8, 2018.  These conditions resulted in exposure to being struck-by the hoisted loads.
