29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):    a)  Trujillo's Cleaning & Painting, 2430 West 82nd Ave, Westminster, CO  80023:  On and before October 19, 2017 the employer did not ensure that each employee working at heights had been trained by a competent person in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii).  Employees accessed roofs and patios at elevations, and had not been trained by a competent person on the fall hazards and the protective measures that are required.  This condition exposed employees to fall hazards.    Abatement note:  The fall protection training areas are referenced as follows:    1926.503(a)(2)(i):  The nature of fall hazards in the work area;    1926.503(a)(2)(ii):  The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used;    1926.503(a)(2)(iii):  The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used;    1926.503(a)(2)(iv):  The role of each employee in the safety monitoring system when this system is used;     1926.503(a)(2)(v):  The limitations on the use of mechanical equipment during the performance of roofing work on low-sloped roofs;    1926.503(a)(2)(vi):  The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection;     1926.503(a)(2)(vii):  The role of employees in fall protection plans;  and     1926.503(a)(2)(viii):  The standards contained in this subpart.

29 CFR 1926.1053(b)(5)(i):  Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  a)  Trujillo's Cleaning & Painting, 2430 West 82nd Ave, Westminster, CO  80023:  On and before October 19, 2017 the employer did not ensure that three non-self-supporting portable ladders were used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support) or 75 degrees.  These ladders were positioned as follows:  1)  23 feet high with the foot positioned 4 feet 4 inches from the wall;  2)  15 feet high with the foot positioned 4 feet 7 inches from the wall; and 3)  12 feet high with the foot positioned 3 feet 8 inches from the wall.  These conditions exposed employees to fall hazards.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders had been trained by a competent person in the proper construction, use, placement, and care in handling ladders:   a)  Trujillo's Cleaning & Painting, 2430 West 82nd Ave, Westminster, CO  80023:  On and before October 19, 2017 the employer did not ensure that each employee using ladders had been trained by a competent person in the proper construction, use, placement, and care in handling ladders.  Employees had not been trained by a competent person on ladder safety such as, but not limited to, the spacing of the foot of the ladder to the top support l(wall), securing the ladder base on sloped roofs, the proper positioning of the ladders' top side rails, and when the use of a spotter is necessary.  This condition exposed employees to fall hazards.
