29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On and before October 19, 2017, in Stuffing Machine Area of the facility, employees were exposed to serious injuries to their hands and or fingers, in that the Pusher Rods near the end of the Stuffer Magazines on the Stuffer Machine, was not guarded to prevent incidental contact with the moving parts. As a result, one employee received an amputation injury to right index finger.
