Location - 5 Water Street, Danvers, MA. On or about Oct 21, 2017, employees were not protected from falling when using a personal fall protection system which did not have an anchor point of supporting at least 5000 pounds. Anchors were not secured per manufacturers instructions. Multiple employees were also attached to two of the four anchors.
