29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Norfolk Naval Shipyard, Scotts Center Annex, Building 1717 Deldonna St., Portsmouth, VA 23709: On or about October 20, 2017, and times prior thereto, the employer did not implement use of an energy control procedural element, where authorized facility employees failed to apply a lockout device to equipment, prior to disassembly and cleaning of equipment including (but not limited to),    a) a Biro band saw, S/N 49495,    b) a Biro band saw, S/N 49496,     c) a Hollymatic 120 Grinder/Bulker, S/N 7005,    d) a Hollymatic 120 Grinder/Bulker, S/N 7030, where the facility energy control procedures were used on a frequent/daily basis, exposing employees to electrical hazards.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  Norfolk Naval Shipyard, Scotts Center Annex, Building 1717 Deldonna St., Portsmouth, VA 23709: On or about October 20, 2017, and times prior thereto, authorized facility employees did not apply a lockout device for energy control, prior to disassembly and cleaning of equipment including,  a) a Biro band saw, S/N 49495,  b) a Biro band saw, S/N 49496,   c) a Hollymatic 120 Grinder/Bulker, S/N 7005,  d) a Hollymatic 120 Grinder/Bulker, S/N 7030, where the facility energy control procedures were used on a frequent/daily basis, exposing employees to electrical hazards. 29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    a) Norfolk Naval Shipyard, Scotts Center Annex, Building 1717 Deldonna St., Portsmouth, VA 23709: On or about October 20, 2017, and times prior thereto, the employer did not provide training to employees on the purpose and use of locks for electrical energy control, where facility employees used energy control procedures frequently/daily for meat department equipment, exposing employees to electrical hazards.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    Norfolk Naval Shipyard, Scotts Center Annex, Building 1717 Deldonna St., Portsmouth, VA 23709: On or about October 20, 2017, and times prior thereto, the employer did not ensure that employees that used hazardous chemicals such as (but not limited to) sodium hypochlorite and sodium hydroxide-containing Suma Block Whitener D10.9 to wash white boards in the meat department, wore appropriate eye protection, exposing employees to potential eye irritation and injury.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
